Breede River Hospice focuses on providing Palliative Care to terminally ill patients and has grown to now offer a range of life-giving and supporting services that, these days, go far beyond what hospice is traditionally known for. 

The Hospice seeks to appoint a Social Worker to be based in the Western Cape.

Salary: R10 124 per month

Duration: 30 hours per week, (this may be negotiated to full time or half day depending on applicant – please state this request on your application)

The purpose of this position is to provide compassionate and professional palliative care psycho social support to patients and their families as a part of the inter-disciplinary care team.


Patient and family care

  • Holistic psycho-social and spiritual assessment of all patients
  • Develop and implement care plans for patients
  • Identify and coordinate groups as appropriate
  • Advise, educate and support families and friends of patients
  • Assist with patient transport and the distribution of equipment e.g. wheelchairs as necessary
  • Advocate for patients regarding funeral policies, insurance etc
  • Facilitate access to resources e.g. SASSA
  • Coordinate group and attend programme “Day Hospice”
  • Meet with volunteers at least quarterly
  • Bereavement care of families
  • Arrange and lead annual remembrance service

 Inter-disciplinary team (IDT)

  • Actively participate in the IDT
  • Refer patients to other members of the IDT
  • Liaise with government departments and other NPO’s
  • Liaise with allied health care works 

 Professional responsibilities

  • Maintain registration with SACSSP
  • Continual professional development on social work, palliative care and IDT
  • Adhere to code of professional ethics
  • Provide training as requested
  • Participate in clinical audits & quality improvement programmes

 Administrative responsibilities

  • Adhere to policies and procedures of Breede River Hospice
  • Complete statistics and reports
  • Participate in staff and patient care meetings
  • Promote the work of hospice in the community.
  • Participate in strategic planning and budgeting


  • Qualification as a Social Worker
  • Registered with SACCSP
  • Minimum of two years’ experience as a social worker
  • Drivers license
  • It would be preferable if the person has experience and / or training in palliative care
